First Consultation
If you are a new patient, please complete a Patient Information Sheet prior to your appointment. This can be completed at the rooms, filled in online, or downloaded/printed and brought with you to your appointment.
For your first consultation with Dr Hewitt, please bring:
- A referral from your doctor or physio, if you have one
- Any films (scans, x-rays etc.) of your knee/shoulder
- Your Medicare and private health fund card
Parking Information
Free parking on Williams road or Portland st Nedlands. Plus 2 hour parking across the Highway.
Public Transport
A bus stop is right outside the door on Stirling Hwy buses include 102, 103, 998 and 999.
